Medical Home
A patient-centered medical home (PCMH) offers a model that puts the patient first. This approach is more holistic, offering patients integrated care that focuses on their health care goals. Health centers recognized as PCMHs can deliver improved quality, efficiency and effectiveness of care.
The Agency for Healthcare Research and Quality (AHRQ) defines a PCMH as having five distinctive features: comprehensive care, patient-centered, coordinated care, accessible service, and quality and safety.
